Vistagen Reveals Top-Line and Post-Hoc Analysis Data from Phase III Trial for Social Anxiety Disorder Treatment

Vistagen has recently announced the top-line results and post-hoc analysis data from its PALISADE-4 Phase III public speaking challenge trial evaluating fasedienol nasal spray for the acute treatment of social anxiety disorder. This trial is a key component of Vistagen's registration-directed PALISADE Phase III program.

According to financial report information disclosed in June 2026, the randomized portion of the PALISADE-4 trial was recently completed, with top-line results announced that same month. Prior to this, Vistagen had announced the top-line results for PALISADE-3 in December 2025, which did not meet its primary endpoint of change from baseline in least squares mean as measured by the Subjective Units of Distress Scale. However, the safety profile of fasedienol in PALISADE-3 was consistent with prior studies, with no drug-related serious adverse events reported.

Against this backdrop, Vistagen optimized the statistical analysis plan for PALISADE-4, incorporating the pre-dose SUDS score at baseline as a pre-specified covariate in the primary efficacy analysis. The company believes that, while there is no guarantee these adjustments will lead to a positive study outcome, the optimized analysis plan aligns with FDA guidance on covariate adjustment in randomized clinical trials. The company is collaborating with external partners to conduct a comprehensive analysis of all available PALISADE program datasets using artificial intelligence and machine learning methods, aiming to further refine the PALISADE-4 statistical analysis plan and regulatory strategy.

Vistagen emphasized that PALISADE-2 previously achieved positive results. Should PALISADE-4 be successful, combined with the positive data from PALISADE-2, it could provide substantial evidence of efficacy for fasedienol in treating social anxiety disorder, supporting the submission of a New Drug Application to the FDA.

Social anxiety disorder affects over 30 million adults in the United States, and there are currently no FDA-approved acute treatments. Fasedienol is a potential first-in-class drug that works by modulating the olfactory-amygdala fear and anxiety neural circuit, without requiring systemic absorption or entry into the brain.
