Merck & Co. has announced a comprehensive early access program for its investigational drug Alimatravir (MK-8527), which is still in Phase III clinical development. The program aims to ensure rapid, widespread, and sustainable availability in low- and middle-income countries if the drug receives regulatory approval.

Alimatravir is a novel, once-monthly oral HIV-1 pre-exposure prophylaxis (PrEP) candidate that becomes effective within one hour of dosing, with a single tablet providing up to one month of protection. The core of this initiative involves signing licensing agreements with generic manufacturers ahead of the completion of Phase III trials to accelerate subsequent distribution.

Key elements of the plan include:

Early voluntary licensing agreements: Merck has entered into royalty-free, non-exclusive voluntary licensing agreements with seven generic drug companies, covering 129 low- and middle-income countries that account for the vast majority of new HIV diagnoses globally. These seven companies include three based in sub-Saharan Africa (Aspen Pharmacare, Quality Chemical Industries, and UCL Kenya) and four based in India (Aurobindo, Cipla, Emcure, and Viatris). This marks the first time in HIV prevention that such licensing agreements have been reached before the completion of Phase III trials, setting a precedent, and is also the first time generic manufacturers from sub-Saharan Africa have been included in the initial licensing round.

Support for regional manufacturing: The plan supports the establishment of regional production capacity in Africa and Latin America to enhance supply resilience and ensure long-term accessibility. Additionally, Unitaid has announced it will provide support to three of the African manufacturers, helping them prepare for potential regional production of Alimatravir.

Early investment in product manufacturing: Merck is investing in its own production capacity ahead of time to ensure an initial supply can be immediately delivered to high-demand regions upon approval, avoiding a supply gap while generic manufacturers ramp up.

Merck Chairman and CEO Robert M. Davis stated that this move reflects the company's deep legacy in HIV and long-term commitment to public health, representing years of collaboration with the HIV community and global health stakeholders. Alimatravir is currently being evaluated in two Phase III clinical trials (EXPrESSIVE-10 and EXPrESSIVE-11), targeting women and adolescent girls in sub-Saharan Africa and other high-risk populations across 16 additional countries, with both trials ongoing.
