Vitasora Health (ASX:VHL) entered a value-based care contract with Iris Medical Group, integrating Vitasora's connected care management services, including remote patient monitoring and chronic care management, into Iris Medical's model of care, according to a Thursday Australian bourse filing.
The first patient enrollments under the program are scheduled to begin in mid-November. Vitasora and Iris Medical expect to enroll between 500 and 1,000 new patients by the end of the year.
Vitasora expects to generate fee-for-service revenues from $50 per patient per month to $90 per patient per month. The contract has a 12-month term, automatically renewed at the end of the term.
