FDA Grants Fast Track Status for Xellsmart's Parkinson's Cell Therapy

XellSmart's proprietary off-the-shelf iPSC-derived cell therapy, XS411, has received Fast Track designation from the U.S. Food and Drug Administration (FDA), marking a significant milestone in its global clinical development.

On July 28, 2026, XellSmart announced that its allogeneic, ready-to-use, injectable iPSC-derived cell therapy candidate, XS411, targeting Parkinson's disease—the second most common neurodegenerative disorder worldwide—has been formally granted Fast Track status by the FDA. This regulatory designation underscores the U.S. health agency's recognition of XS411's clinical value and its potential to address the significant unmet medical needs in Parkinson's disease treatment, serving as a crucial milestone for the product's global clinical development and future market access.

How Fast Track Status Accelerates Clinical Development

Fast Track designation is an expedited review pathway established for investigational drugs targeting serious conditions with unmet medical needs. With this designation, XS411 becomes eligible for a series of regulatory benefits throughout its U.S. development and commercialization process.

First, the team can engage in regular, in-depth discussions with FDA reviewers throughout the drug development process to optimize clinical trial design and adjust research strategies. Second, XS411 qualifies for a rolling Biologics License Application (BLA) review, which can significantly shorten regulatory review timelines. Third, upon achieving specified clinical endpoints, XS411 may be eligible for FDA Priority Review, further accelerating its market approval in the United States. These regulatory advantages will collectively accelerate the clinical advancement, regulatory review, and commercial launch of XS411 in the U.S. market.

The achievement of this Fast Track status builds on the solid clinical progress of XS411 since 2025, when the product received IND approval from the FDA in January 2025, along with several waivers. This new designation further validates the therapeutic potential of XS411 to address the unmet medical needs of Parkinson's disease patients worldwide.

Clinical Progress: XS411 Completes Full Enrollment in Multicenter Phase II Trial

In July 2026, XellSmart announced that the multicenter Phase II registrational trial of XS411 for Parkinson's disease has completed full patient enrollment. The trial employs the globally recognized PROBE design (Prospective, Randomized, Open-label, Blinded Endpoint Evaluation), which provides stronger clinical evidence compared to traditional single-arm open-label studies. This trial design meets the rigorous clinical implementation requirements of cell therapy while reducing study bias through randomization and blinded endpoint assessment, thereby enhancing overall trial rigor. This optimized design is more likely to generate robust, credible efficacy data to support the program's subsequent Phase III registrational development and approval.

The advancement of the Phase II trial builds on encouraging Phase I results. Cumulative long-term follow-up data indicate that the cell therapy product has a favorable safety and tolerability profile. Following transplantation, subjects achieved statistically significant improvements in MDS-UPDRS Part III motor scores during OFF periods, along with extended ON time duration and improved quality of life metrics compared to baseline. Molecular imaging confirmed that the transplanted cells successfully engrafted, survived, and differentiated within the brain parenchyma, acquiring the functional capacity to synthesize and secrete the neurotransmitter dopamine.

To date, XellSmart has secured IND approvals for multiple registrational clinical trials. The company holds a global leadership position in developing iPSC-derived cell therapies for neurological disorders, including Parkinson's disease and amyotrophic lateral sclerosis (ALS). With the steady progression of its registrational programs, this cell therapy, which has the potential to both alleviate symptoms and modify disease progression, promises to offer innovative treatment options for the more than 10 million Parkinson's disease patients worldwide.
