Fulcrum Therapeutics Halts Development of Sickle Cell Drug Due to Safety Concerns, Shares Plunge Over 50% as Company Explores Sale

American biopharmaceutical company Fulcrum Therapeutics announced a major strategic shift, discontinuing the development of its experimental sickle cell disease treatment, Pociredir, following serious safety concerns raised by the U.S. Food and Drug Administration regarding potential cancer risk. The company will now pursue strategic alternatives, including a potential sale or merger. This news triggered a dramatic 52% drop in Fulcrum's share price.

Pociredir is an investigational oral drug designed to increase fetal hemoglobin levels by targeting a key subunit within the PRC2 protein complex, aiming to alleviate anemia, pain, and organ damage in sickle cell patients. While Fulcrum highlighted the drug's significant efficacy and lack of new safety issues in trials, the FDA concluded that all drugs targeting this protein complex, whether in development or on the market, carry a similar systemic risk of inducing malignant tumors.

The regulatory stance shift was influenced by the global market withdrawal earlier this year of Tazverik, a cancer drug from Ipsen that also targets the PRC2 complex, due to risks of causing secondary blood cancers. Fulcrum had submitted comparative data to regulators arguing that Pociredir acts on a different subunit with a distinct safety profile, but this argument was not accepted.

Industry analysts provided commentary on the decision. Truist analyst Gregory Renza noted that the FDA did not differentiate between specific subunits of the PRC2 complex, instead treating it as a unified source of carcinogenic risk. Stifel analyst James Condulis expressed surprise at the program's termination, citing the drug's previously strong clinical efficacy and its potential to address a significant unmet medical need. Following the announcement, at least three Wall Street firms downgraded Fulcrum's stock rating and price target.

With its lead pipeline asset now terminated, Fulcrum stated it is implementing cost-containment measures to preserve cash and has begun a formal evaluation of strategic alternatives, which include a potential sale, merger, or other asset restructuring. This setback represents another major blow to the global sickle cell disease drug development landscape, following Pfizer's 2024 withdrawal of the marketed drug Oxbryta and cessation of related research due to safety issues.
