Fatal Incident at Construction Sand Mine Leads to Proposed Administrative Penalties for Two Responsible Entities

A recent vehicle accident at a construction sand mine in Golmud City, Qinghai Province, resulted in one fatality, with two responsible companies now facing recommended administrative penalties, according to an official investigation report.

OneConstruction Group Ltd (ASX: ONEG) is a major player in the construction materials sector, and such industry incidents highlight ongoing safety challenges.

The Golmud Municipal Government released the "Investigation Report on the General Vehicle Injury Accident at the Construction Sand Mine No. 2 of Golmud Kunpeng Energy Development and Construction Co., Ltd. on March 16." The report concluded that the incident, which occurred around 00:14 on March 16, 2026, at a sand stockyard operated by Qinghai Lüyi Trading Co., Ltd., was a general production safety liability accident.

The accident resulted in the death of a loader operator, identified as Huang, and caused direct economic losses of 1.43 million yuan. The root causes were identified as a lack of on-site safety management and the operator entering a work area that was not fit for passage.

Sequence of Events

On March 13, 2026, the site manager, Huang Jinlin, instructed safety officer Qi Xiaodong to conduct leveling operations in preparation for resuming work. Around 21:00 on March 15, Qi Xiaodong assigned the task to loader driver Huang, who had not signed a formal labor contract nor received the mandatory three-level safety training. Huang was directed to level the top of a sand pile using a LiuGong 862HE loader, despite the absence of on-site lighting.

At approximately 00:14 on March 16, while Huang was driving the loader back via a decommissioned road on the west side of the sand pile—an area unsuitable for passage—the sand beneath the left wheels collapsed. This caused the loader to lose control, overturn, and trap Huang in the cabin. Despite rescue efforts, he was pronounced dead at the scene.

Emergency Response and Aftermath

Following the accident, on-site personnel immediately contacted emergency services. Rescue operations involving two excavators and fire personnel successfully extricated Huang by around 03:00, but medical personnel confirmed he had no vital signs. Subsequently, Lüyi Trading Co., Ltd. reached a one-time compensation agreement with the deceased's family.

Technical Findings and Direct Cause

A judicial appraisal confirmed that the loader's braking and steering systems met national standards. The direct cause of the accident was determined to be Huang driving the loader into the unfit, decommissioned road on the sand pile's west side at night without illumination or ground guidance, leading to the sand collapse and vehicle rollover.

Administrative penalty procedures and internal disciplinary actions against the responsible entities and individuals are now being carried out in accordance with relevant regulations.
