It was learned from the Lanzhou Municipal Medical Insurance Bureau on September 24 that starting from January 1, 2027, the employer contribution portion of the large medical insurance premium for enterprise retirees citywide will no longer be collected, while the individual contribution rate and payment method will remain unchanged; for flexibly employed retirees, the individual contribution rate for large medical insurance will be halved.

It is reported that large medical insurance is an important supplementary safeguard to basic medical insurance and plays a significant role in reducing the burden of high medical expenses for insured patients with major and critical illnesses. This marks another targeted policy measure by Lanzhou following the decoupling of retiree benefit eligibility from employer contribution status under basic medical insurance, aimed at loosening restrictions and reducing burdens on employers.

After the new policy takes effect, it will bring about positive changes in five aspects.

Optimizing the business environment and reducing enterprise operating and management costs. Employers, especially small and micro enterprises, will no longer pay large medical insurance premiums for retirees, thereby reducing labor costs and continuously optimizing the business environment.

Reducing contribution burdens and benefiting flexibly employed retirees. The individual contribution rate for large medical insurance for flexibly employed retirees will be adjusted from 0.5% to 0.25%, reducing the per capita annual contribution burden by 226.32 yuan, effectively easing the economic burden on insured individuals.

Deepening decoupling and promoting the socialized management of retirees. After the policy takes effect, retirees' eligibility for medical insurance benefits will no longer be affected by their employer's large medical insurance contribution status. Employers will no longer need to retroactively collect large medical insurance premiums from individuals, better safeguarding retirees' medical insurance rights.

Releasing accumulated funds and promoting dynamic balance in fund revenue and expenditure. After the new policy takes effect, the annual surplus rate of the large medical insurance fund will drop to a reasonable range, which is conducive to releasing accumulated fund surpluses and promoting the stable operation of the fund.

Improving administrative efficiency and ensuring smooth and efficient convenience services. After insured employees transition from active employment to retirement under basic medical insurance, the employer contribution portion of large medical insurance will be automatically waived, truly achieving "enjoyment without application."
