The Changsha Municipal Government official website published the "Investigation Report on the 'November 12' Deflagration Accident at Liuyang Yihelong Fireworks Manufacturing Co., Ltd." on February 3rd. At approximately 9:24 AM on November 12, 2025, a combustion accident occurred in the empty shell tail application workshop (Building 146) of Liuyang Yihelong Fireworks Manufacturing Co., Ltd. The fire affected an area of about 140 square meters, resulting in 3 severe injuries, 2 minor injuries, and direct economic losses of 933,000 yuan. The investigation concluded that the 'November 12' deflagration accident at Liuyang Yihelong Fireworks Manufacturing Co., Ltd. was a production safety responsibility accident caused by inadequate risk identification and hidden hazard investigation, illegally contracting maintenance projects to unqualified individuals, and violations of hot work regulations, among other factors.
On November 11, 2025, an instruction to halt production operations was issued via the company's WeChat group for the production line in Area 1 of Yihelong Fireworks Company. On the morning of November 12, welding repair work began on the drainage gutter southwest of the workshop. At around 9:24 AM, welding slag from the electric welding operation fell and splashed onto a chemical barrel (which had residual chemical residue inside that was not cleaned properly) and a conductive rubber mat (which had caked chemical deposits on it) near the workshop entrance, causing a deflagration. This subsequently ignited the tail charge and fuses in the empty shells inside the workshop, injuring 5 personnel on site to varying degrees.
Following the accident, the Changsha city government directed Liuyang city to quickly establish a treatment task force and invited experts for consultations. As of December 25, 2025, one injured person had been discharged from the hospital, while the other four were undergoing wound dressing changes or skin graft treatments, with stable vital signs. The direct cause of the accident was that during the production halt period, before performing hot work for workshop inspection and maintenance, the operators failed to transfer semi-finished products from the production workshop to the transfer area as required and did not strictly follow the operational safety procedures to clean tools and equipment, leading to residual chemical residue in the barrel and caked chemicals on the rubber mat. During the hot work, only a single oilcloth was used to cover the empty shells containing fuses and tail charge, and the tail application chemical barrel was not fully covered. Sparks from the welding splashed onto the tail application chemical barrel, causing the improperly cleaned tail application chemicals inside the barrel and the caked chemicals on the conductive rubber mat to ignite violently, triggering the accident.
The involved maintenance worker (welder), Zhang Moujin, did not possess the special operation certificate for electric welding and illegally performed welding work, leading to the accident. This resulted in the severe consequences of 3 severe injuries, 2 minor injuries, and direct economic losses of 933,000 yuan. He bears direct responsibility for the accident, and his actions are suspected of violating Article 21 of the "Fire Protection Law of the People's Republic of China." It is recommended that the Liuyang public security organs investigate and handle the case according to law. Yihelong Fireworks Company failed to adequately implement its primary responsibility for production safety, did not promptly investigate and eliminate potential safety hazards, provided insufficient safety education and training for inspection and maintenance personnel, and inadequately managed the hot work site, thus bearing responsibility for the accident. It is recommended that the Liuyang Emergency Management Bureau impose administrative penalties in accordance with the "Work Safety Law of the People's Republic of China."
Furthermore, it is recommended that legal actions be taken against four individuals, including Lin Mou, the legal representative of Yihelong Fireworks Company. Clues and related materials concerning issues identified during the accident investigation regarding the performance of duties by public officials in local Party committees, governments, and relevant departments have been transferred to the municipal Commission for Discipline Inspection and Supervisory Commission for investigation and handling.
