Universal Health Services shares tumbled 7.41% in after-hours trading on Monday, as investors reacted negatively to the company's reduced full-year profit forecast, overshadowing better-than-expected second-quarter revenue.

The hospital operator reported Q2 revenue of $4.64 billion, topping analyst estimates of $4.57 billion, with adjusted earnings per share coming in at $5.98. Same-facility acute care hospitals saw adjusted admissions rise 2.9% and net revenue per adjusted admission increase 3.0%, while behavioral health facilities posted a 7.1% rise in net revenue per adjusted admission.

However, the company revised its 2026 adjusted EBITDA forecast lower to a range of $2.610 billion to $2.717 billion, down from its prior outlook. Universal Health also cut its full-year adjusted EPS guidance to $22.28 to $23.65, below its earlier forecast, even as it slightly raised its net revenue projection to $18.501 billion to $18.762 billion. The downward revision in profitability metrics appeared to drive the sharp post-market selloff.
