罚单显示，青海格尔木农村商业银行股份有限公司的主要违法违规行为是：员工行为管理不到位。
针对上述违法行为，海西金融监管分局对青海格尔木农村商业银行股份有限公司罚款30万元；对何向东罚款5.5万元；对牛健和、许正芳分别给予警告；对保英浩禁止从事银行业工作10年。
